Effective Monday, March 23rd, we have moved all client services to telehealth due to the COVID-19 Crisis. We believe this is the safest decision we can make in order to reduce risk to our clients, staff, and community. We use telehealth services through www.doxy.me. Each therapist has their own confidential, secure, and encrypted site that is HIPAA compliant. If you are a current client, please contact your therapist for instructions. You will need to go to the CLIENT FORMS tab and complete the "Teletherapy Consent Form." This form will need to be printed and signed, then returned by FAX at 866-540-3094, scanned or photographed, and attached as an email to your therapist, or sent by mail.
POTENTIAL NEW CLIENTS: If you would like to become a NEW client, please call our office at 574-282-1090, and follow the prompts to leave a message with our office assistant. She will get back to you to determine the reason for your call, check availability of therapists, and obtain your insurance information. She will also give you instructions to complete a "New Client Information form online. Once you have been scheduled, you will need to complete all the Consent Forms by going to the CLIENT FORMS tab. Please print, sign and return via FAX at 866-540-3094, or send by regular mail.
